A note to my Clients: Before we start working together, I want you to feel informed, supported, and totally clear on how your information is handled and what you can expect from therapy. These documents—your Privacy Policy, Terms of Service, and Informed Consent—are here to protect you, outline your rights, and make sure we’re on the same page from the start.
I know policies aren’t the most exciting part of therapy, but they’re important. They help create transparency, safety, and trust in our work together.
If anything is unclear or you want to talk something through, you’re always welcome to ask questions—this is your space, and you deserve to understand every part of the process.
Privacy Policy

Stephanie Annesley (“I,” “me,” or “my practice”) is committed to protecting your personal information and personal health information. This Privacy Policy outlines how your information is collected, used, stored, and disclosed when you receive services from Stephanie Annesley, whether online or in person.
This policy is designed to comply with Ontario’s Personal Health Information Protection Act (PHIPA), the Personal Information Protection and Electronic Documents Act (PIPEDA), and other applicable Canadian privacy requirements.

There is risk of serious harm to you or others
A child may be in need of protection

Research is conducted under strict ethics board approval
4.3 With Service Providers
I may use trusted third-party services such as:
Secure video platforms
Cloud storage
Payment processors
These providers are required to protect your information and follow privacy regulations.
5. How Your Information Is Protected

6. How Long Information Is Kept
Your records are kept according to regulatory and legal standards:
Adult records: Stored for at least 10 years after your last appointment
Records for minors: Stored for 10 years after the client turns 18
Financial/administrative records: Kept for a minimum of 7 years
Electronic communications: Stored in accordance with platform requirements
After the required period, records are securely destroyed or deleted.
7. Your Rights
You have the right to:
7.1 Access Your Record
Request access to your personal information or PHI.
7.2 Request Corrections
Ask that outdated or inaccurate information be corrected.
7.3 Manage Your Consent
Change or withdraw consent for certain uses or disclosures.
7.4 Express Concerns
You may file a concern directly with the practice or with the Information and Privacy Commissioner of Ontario.
8. Working With Minors
When working with individuals under 18:
Capacity to consent is assessed
Parent/guardian consent is obtained when required
Privacy rights are explained in developmentally appropriate ways
Information is only shared with parents/guardians as permitted by law
9. Cross-Border Use of Technology
Your information is stored in Canada unless otherwise stated.
If any service providers store information outside Canada, they are required to follow strong privacy protections comparable to Canadian standards.
No PHI will be transferred outside Canada without your express consent unless required for healthcare or legal reasons.
10. Privacy Breaches
If a privacy breach occurs, I will:
- Notify you if there is a risk of harm
- Report the breach to the Information and Privacy Commissioner (if required)
Take steps to contain and investigate the breach
Document what happened and how it was resolved
11. Changes to This Policy
This policy may be updated periodically. Any significant updates will be posted and the “Last Updated” date will change. Additional consent may be requested if changes affect how PHI is handled.

IMPORTANT EMERGENCY INFORMATION
If you are experiencing an emergency or an immediate risk of harm, call 911 or go to the nearest emergency department.
If you are thinking about suicide, considering harming yourself or someone else, or feel unable to keep yourself safe, you must contact emergency services or crisis support immediately. These Terms of Service do not replace crisis intervention services.
1. Agreement to the Terms
These Terms of Service (“Terms”) form an agreement between you (the client) and Stephanie Annesley (“the therapist,” “I,” “me,” or “my practice”). By accessing or using Jane App (“the Platform”), you agree to be bound by these Terms.
The Platform may include online therapy sessions, intake forms, messaging features (if applicable), scheduling tools, and other mental health resources.
If you do not agree to these Terms, please do not use the Platform.
2. Eligibility for Services
By using the Platform, you confirm that:
You are 18 years of age or older (or the age of majority in your province/territory)
You have the legal authority to enter into this agreement
All information you provide is accurate and complete
You will use the Platform only for lawful and permitted purposes
If accessing on behalf of another person, you have legal authority to do so
3. Description of Services
3.1 Therapy Services
I provide regulated psychotherapy and counselling services, which may include:
Individual psychotherapy
Mental health assessments
Treatment planning
Psychoeducation
Referrals when appropriate
3.2 Informational Resources
Any workshops, guides, or educational materials offered through the Platform are for informational purposes only and are not a substitute for therapy or medical treatment.
4. Geographic Limitations
Services are available only when you are physically located in Ontario, Canada, unless otherwise permitted by regulation.
By using the Platform, you confirm you will be in Ontario during all therapy sessions.
5. Account Creation and Information
To access services, you may need to create an account and provide accurate information.
You agree not to:
Create more than one account
Share your login credentials
Create an account on someone else’s behalf without authorization
You are responsible for maintaining the security of your account.
6. Fees & Payments
You agree to pay all session fees in accordance with the rates provided to you.
Payment is due on or after each session, and a valid payment method must be kept on file.
By providing payment information, you authorize Stephanie Annesley to process charges for completed sessions.
If fees change, clients will be notified in advance.
7. Cancellations & Missed Appointments
Appointments cancelled with less than 24 hours’ notice may be subject to the full session fee.
Missed appointments (“no-shows”) may also result in a charge.
Exceptions may be made for emergencies at the therapist’s discretion.
8. Privacy & Confidentiality
Your personal information and personal health information are handled in accordance with:
Ontario’s Personal Health Information Protection Act (PHIPA)
Applicable federal privacy laws

By using the Platform, you acknowledge that your information will be collected, used, and stored according to these policies.
9. Technology Use & Limitations
Online therapy involves risks such as:
Technical difficulties
Service interruptions
Possible privacy breaches beyond reasonable control
If a disruption prevents a session from continuing, we will attempt to reconnect or reschedule.
10. Disclaimers
The Platform and any materials provided are offered “as is” and subject to availability.
I do not guarantee uninterrupted access, error-free functionality, or that online tools will meet all expectations.
Therapy outcomes cannot be guaranteed.
11. Limitation of Liability
To the fullest extent permitted by law:
Your sole remedy for dissatisfaction with the Platform is to stop using it.
I am not liable for indirect, incidental, or consequential damages.
I am not responsible for issues arising from third-party providers or technology failures.
This limitation does not apply where prohibited by Ontario law.
12. Ending Services
Either you or I may end the therapeutic relationship at any time.
If services end:
You will be informed of appropriate next steps or referrals.
Your clinical record will be retained for the period required by regulation (typically 10 years from the last session, or 10 years after the client turns 18 for minors).
13. Resolving Disputes
If a disagreement arises, both parties agree to:
Discuss the issue directly and in good faith
Attempt informal resolution
Consider mediation if needed
Legal disputes will be governed by Ontario law.
14. Governing Law
These Terms are governed by:
The laws of the Province of Ontario
Applicable federal laws of Canada

1. General Information
Therapy is both a personal process and a formal agreement. This document explains how we will work together, what you can expect, and your rights as a client.
By signing at the end, you acknowledge that you understand this information and provide consent to begin therapy.
2. Confidentiality
Your privacy matters. All personal health information is protected under Ontario’s Personal Health Information Protection Act (PHIPA).
Your session notes, intake information, and communication records are stored in a secure, PHIPA-compliant digital system and are not shared without your consent except in the specific situations listed below.
3. Limits to Confidentiality
There are rare circumstances where I am legally or ethically required to share information:
Suspected abuse or neglect of a child or vulnerable person
Serious and imminent risk of harm to yourself or someone else
A court order requiring disclosure
Disclosure of abuse by a regulated health professional
Couples Therapy
In couples therapy, confidentiality is shared between partners. I cannot keep one partner’s information private from the other, and both must consent to any disclosures.
4. Risks and Benefits of Therapy
Therapy can improve emotional wellbeing, clarity, coping skills, and self-awareness. At times, exploring difficult topics may bring up uncomfortable emotions.
If you ever feel overwhelmed or unsure, please let me know so we can adjust the pace or approach.
5. Clinical Supervision – Working With a Registered Psychotherapist (Qualifying)
As a Registered Psychotherapist (Qualifying) with the College of Registered Psychotherapists of Ontario (CRPO), I am required to receive ongoing clinical supervision until I am eligible to transfer to full RP status.
Supervision Involves:
Regular consultation with a qualified Clinical Supervisor
Discussion of clinical themes, interventions, and treatment planning
Maintaining strict confidentiality and PHIPA compliance at all times
You will be provided with the name and credentials of my Clinical Supervisor during your first session.
6. Session Recordings (If Applicable)
With your written consent only, sessions may be recorded for supervision and learning purposes.
Recordings are used only for supervision or training
They may be viewed by a small, authorized peer or supervision group if required
They are stored securely on PHIPA-compliant platforms
They are permanently deleted at the end of the supervision period or academic term
You may decline or withdraw consent at any time
Recordings are never used without prior discussion and written permission.
7. Record Keeping
I am required to keep brief clinical notes summarizing each session. Notes may include themes, observations, or therapeutic interventions.
Records are:
Stored securely in a PHIPA-compliant digital system
Retained for 10 years after your last session (or 10 years after age 18 for minors)
Accessible to you upon request (a small administrative fee may apply)
No administrative staff or third parties have access to your records unless discussed with you and agreed upon.
8. Social Media & Boundaries
To protect your privacy and maintain clear therapeutic boundaries:
I do not accept friend requests or follow clients on social media (e.g., Instagram, Facebook, LinkedIn)
I do not communicate through personal accounts
If you have questions about digital boundaries, please feel free to ask.
9. Standards of Practice
I follow the Standards of Practice and Code of Ethics set by the College of Registered Psychotherapists of Ontario.

10. Remote & Electronic Therapy
Most sessions are offered through secure, encrypted, PHIPA-compliant video platforms.

Although every reasonable measure is taken to protect your information, no digital method can be guaranteed 100% secure. You may pause or discontinue online therapy at any time.
11. Technology Requirements
For virtual sessions, you will need:
A private, quiet location
Reliable internet connection
A device with a camera and microphone
A phone available as backup
You will be asked to provide your location and phone number at the start of each session in case of emergency or technical interruption.
12. Appointment Times
Standard sessions are 50 minutes. Extended sessions (e.g., 75 minutes) may be available upon request.
Frequency and duration of therapy will be determined collaboratively based on your needs and goals.
13. Fees & Payment
You will receive a receipt after each session, which includes:
My name and CRPO registration number
My Supervisor’s registration number (if required by your insurance provider)
Additional letters, summaries, or forms may be provided upon request for a fee of $45.
14. Missed or Cancelled Sessions
Sessions cancelled with less than 24 hours’ notice may be charged the full session fee
Missed sessions (“no-shows”) are billed in full
Receipts for missed sessions indicate that the appointment was unattended
Exceptions may be made in emergencies at my discretion.
15. Emergencies & Crisis Situations
I am not available for crisis support outside of scheduled sessions.
If you are in crisis or immediate danger, please contact:
- 911
- Nearest emergency department
- 9-8-8 Suicide & Crisis Helpline (24/7 across Canada)
16. Communication Outside Sessions
Email is used for scheduling and administrative purposes only.
Please do not send sensitive personal or clinical information via email.
Documents sent via email may be password-protected for your privacy.
For couples therapy, communication may be shared with both partners to maintain transparency.
17. Complaints
If you have concerns about your therapy experience, you are encouraged to raise them with me directly so we can address them together. If you prefer to speak with someone else or wish to file a complaint, you may contact:
