IMPORTANT EMERGENCY INFORMATION
If you are experiencing an emergency or an immediate risk of harm, call 911 or go to the nearest emergency department.
If you are thinking about suicide, considering harming yourself or someone else, or feel unable to keep yourself safe, you must contact emergency services or crisis support immediately. These Terms of Service do not replace crisis intervention services.
1. Agreement to the Terms
These Terms of Service (“Terms”) form an agreement between you (the client) and Stephanie Annesley (“the therapist,” “I,” “me,” or “my practice”). By accessing or using Jane App (“the Platform”), you agree to be bound by these Terms.
The Platform may include online therapy sessions, intake forms, messaging features (if applicable), scheduling tools, and other mental health resources.
If you do not agree to these Terms, please do not use the Platform.
2. Eligibility for Services
By using the Platform, you confirm that:
You are 18 years of age or older (or the age of majority in your province/territory)
You have the legal authority to enter into this agreement
All information you provide is accurate and complete
You will use the Platform only for lawful and permitted purposes
If accessing on behalf of another person, you have legal authority to do so
3. Description of Services
3.1 Therapy Services
I provide regulated psychotherapy and counselling services, which may include:
Individual psychotherapy
Mental health assessments
Treatment planning
Psychoeducation
Referrals when appropriate
3.2 Informational Resources
Any workshops, guides, or educational materials offered through the Platform are for informational purposes only and are not a substitute for therapy or medical treatment.
4. Geographic Limitations
Services are available only when you are physically located in Ontario, Canada, unless otherwise permitted by regulation.
By using the Platform, you confirm you will be in Ontario during all therapy sessions.
5. Account Creation and Information
To access services, you may need to create an account and provide accurate information.
You agree not to:
Create more than one account
Share your login credentials
Create an account on someone else’s behalf without authorization
You are responsible for maintaining the security of your account.
6. Fees & Payments
You agree to pay all session fees in accordance with the rates provided to you.
Payment is due on or after each session, and a valid payment method must be kept on file.
By providing payment information, you authorize Stephanie Annesley to process charges for completed sessions.
If fees change, clients will be notified in advance.
7. Cancellations & Missed Appointments
Appointments cancelled with less than 24 hours’ notice may be subject to the full session fee.
Missed appointments (“no-shows”) may also result in a charge.
Exceptions may be made for emergencies at the therapist’s discretion.
8. Privacy & Confidentiality
Your personal information and personal health information are handled in accordance with:
Ontario’s Personal Health Information Protection Act (PHIPA)
Applicable federal privacy laws

By using the Platform, you acknowledge that your information will be collected, used, and stored according to these policies.
9. Technology Use & Limitations
Online therapy involves risks such as:
Technical difficulties
Service interruptions
Possible privacy breaches beyond reasonable control
If a disruption prevents a session from continuing, we will attempt to reconnect or reschedule.
10. Disclaimers
The Platform and any materials provided are offered “as is” and subject to availability.
I do not guarantee uninterrupted access, error-free functionality, or that online tools will meet all expectations.
Therapy outcomes cannot be guaranteed.
11. Limitation of Liability
To the fullest extent permitted by law:
Your sole remedy for dissatisfaction with the Platform is to stop using it.
I am not liable for indirect, incidental, or consequential damages.
I am not responsible for issues arising from third-party providers or technology failures.
This limitation does not apply where prohibited by Ontario law.
12. Ending Services
Either you or I may end the therapeutic relationship at any time.
If services end:
You will be informed of appropriate next steps or referrals.
Your clinical record will be retained for the period required by regulation (typically 10 years from the last session, or 10 years after the client turns 18 for minors).
13. Resolving Disputes
If a disagreement arises, both parties agree to:
Discuss the issue directly and in good faith
Attempt informal resolution
Consider mediation if needed
Legal disputes will be governed by Ontario law.
14. Governing Law
These Terms are governed by:
The laws of the Province of Ontario
Applicable federal laws of Canada

By booking an appointment, creating an account, or checking the agreement box, you confirm that you:

Understand them
Agree to be bound by them
